BELLA ART JIU JITSU ACADEMY

WAIVER, RELEASE OF LIABILITY & MEMBERSHIP AGREEMENT

By signing this Agreement, I acknowledge that Brazilian Jiu-Jitsu, No-Gi, Wrestling, Self-Defense, Fitness Training, and all related activities involve physical contact and carry inherent risks, including but not limited to serious injury, permanent disability, illness, or death. I voluntarily choose to participate and accept all risks associated with these activities.

I release and hold harmless Bella Art Jiu Jitsu Academy, its owners, instructors, coaches, employees, volunteers, and affiliates from any claims, liabilities, injuries, damages, or losses arising from participation in classes, open mats, seminars, competitions, or any academy-related activities, except where prohibited by applicable law.

I certify that I (or my child) am physically able to participate in training. I understand it is my responsibility to consult a physician before participating if I have any medical condition that could affect my ability to train. In the event of an emergency, I authorize Bella Art Jiu Jitsu Academy to obtain emergency medical treatment if I cannot be reached.

I understand that Brazilian Jiu-Jitsu includes throws, takedowns, joint locks, chokeholds, grappling, and other techniques that may result in injury despite proper supervision. I agree to train responsibly, follow all instructions, and immediately inform an instructor of any injury or medical condition.

I agree to follow all academy rules, including maintaining proper hygiene, keeping fingernails and toenails trimmed, wearing clean training gear, removing shoes before entering the mats, treating instructors and teammates with respect, and refraining from unsafe or inappropriate behavior. I understand that Bella Art Jiu Jitsu Academy reserves the right to suspend or terminate any membership due to misconduct or failure to follow academy policies.

I understand that Gi classes require the official Bella Art Jiu Jitsu Academy uniform unless otherwise authorized by the academy.

I understand that tuition is billed automatically according to my membership agreement. Membership cancellations require a written 30-day notice. No refunds will be issued for tuition already paid except where required by law.

I understand that Bella Art Jiu Jitsu Academy is not responsible for lost, stolen, or damaged personal property.

I authorize Bella Art Jiu Jitsu Academy to photograph or record me (or my child) during classes, seminars, competitions, and academy events. I grant permission for these images or videos to be used for promotional, educational, and marketing purposes without compensation.

I agree not to attend classes while experiencing symptoms of any contagious illness. I understand that participation in group activities carries a risk of exposure to viruses, bacteria, and other communicable diseases, and I voluntarily assume that risk.

This Agreement shall be governed by the laws of the Commonwealth of Massachusetts. If any provision is found unenforceable, the remaining provisions shall remain in full force and effect.

By signing below, I acknowledge that I have carefully read this Agreement, fully understand its contents, and voluntarily agree to all of its terms.
